Registration

For 2023, we have concluded contracts with the following health insurance providers and their labels: DWS, ASR, Zorg en Zekerheid, Caresq, Zilveren Kruis and Menzis.

If you would like your care to be covered by your health insurance then a referral letter from your GP will be required. Once you have obtained a letter from your GP referring you to general basic care (BGGZ), you may then fill in the registration form below.

If you will not be claiming your sessions from your health insurer, for example when you or your employer will be paying for your treatment, then you may also register with our practice directly by clicking the “register” button below. We will contact you as soon as possible to make an initial appointment.
